Best Practice Day October 2021 - videos now available

AWTTC successfully hosted its second virtual Best Practice Day on the 12 October 2021.

This was an online event focused on sharing some of the excellent medicines optimisation work ongoing in Wales, and provided the opportunity for attendees to ask any questions they may have.

Rachel describes the way her practice has changed how it manages chronic disease, involving the whole practice. It required a significant shift of mindset and well led team ownership to maximise its efficiencies. The initiative centres around an annualised medication review triggered by patient date of birth and has proven very successful in helping to reduce the complexity of the clinical management system. The result is provision of timely, efficient, prudent, holistic and high-quality patient reviews utilising the whole practice team.

Safer Prescribing of Sodium Valproate in Primary Care
Rory Wilkinson, Head of Pharmacy, Primary Care and Community Services (Central) for Betsi Cadwaladr University Health Board (UHB)

Rory talks about how they identified that there were extant inconsistencies with sodium valproate safety measures within BCUHB primary care and how they developed a programme to try and manage the outstanding risks. Rory’s presentation includes the history of valproate, the audit they developed, their communication and education strategy and the many challenges around this successful initiative.
